2. Definitions
- Practice — Indywidualna Praktyka Lekarska Katarzyna Zugaj.
- Patient — a person using health services provided at the Practice.
- Services — the health services listed in the Price list.
- Deposit — the amount paid when booking a first consultation, within the meaning of Art. 394 of the Polish Civil Code (zadatek).
3. Scope of services
The Practice provides outpatient services: medical consultations for hair loss and scalp conditions (including trichoscopy) and general medical consultations for adults. The Practice has no contract with the National Health Fund (NFZ) — all services are paid. The Practice does not provide emergency care, remote consultations or home visits. In a medical emergency, call 112.
4. Website and form
The website hairyu.com contains information about the scope and types of health services provided (Art. 14 of the Medical Activity Act) and is not medical advice. It is not an online shop — there is no basket and no payment on it. The appointment request form is a free electronic service; it prepares an email that you send yourself and requires a browser with JavaScript and an email app. Please do not send information about your health through it or by email. Sending unlawful content is prohibited.
5. Booking
Visits are booked by phone, by email, with the form or in person. A time is reserved once the Practice confirms it (by text, email or in person), and for a first consultation once the deposit is paid. The confirmation states the type of visit, the time, the price and, where applicable, the deposit.
6. Prices and payments
Prices are set out in the Price list; they are given in Polish złoty (PLN), and for guidance in GBP and USD. Services are VAT-exempt under Art. 43(1)(19) of the Polish VAT Act. Visits are paid for at the Practice when they end — by card or in cash. The deposit is paid in advance: online through a secure Stripe link (Visa or Mastercard, Apple Pay, Google Pay, BLIK) or by bank transfer, in PLN, GBP or USD. Online payments are processed by Stripe Payments Europe, Limited, 1 Grand Canal Street Lower, Grand Canal Dock, Dublin, D02 H210, Ireland; the Practice has no access to full card details and passes no health information to the payment provider. Payments appear on statements as HAIRYU GABINET. The Practice issues a fiscal receipt for every visit and an invoice on request. Details: Payments, deposit and refunds.
7. Deposit and cancellation
A first consultation is booked once a deposit of PLN 100 is paid, which is credited towards the price of the visit. The Patient may cancel or move the visit free of charge no later than 24 hours before it — the deposit is then refunded in full or, at the Patient's request, moved to a new date. If the Patient cancels later or does not attend, the Practice may keep the deposit (Art. 394 § 1 of the Polish Civil Code); in the event of sudden illness or an emergency reported by the day of the visit at the latest, the deposit is moved to a new date. If the Practice cancels the visit, it refunds the deposit in full or moves it to a new date, at the Patient's choice, without prejudice to the Patient's rights under Art. 394 of the Polish Civil Code.
8. The visit
Services are provided with due care and in line with current medical knowledge. Examinations are carried out with the Patient's consent and, for minors, with the consent of the legal representative (for patients aged 16 and over, also with the Patient's consent). The Practice sees patients aged 16 and over; general consultations are for adults only. The Patient informs the doctor about medicines, allergies and illnesses. If a Patient is more than 15 minutes late, the visit may be shortened or moved if it cannot be held without delaying other visits; the deposit rules apply accordingly.
9. E-prescriptions, e-referrals, e-sick notes
Prescriptions, referrals and certificates of temporary incapacity for work are issued electronically, only after an examination and where medically indicated. The data needed to issue them is sent to the P1 system run by the e-Health Centre (Centrum e-Zdrowia), and for sick notes to the Social Insurance Institution (ZUS).
10. Patient rights and records
The Practice respects the patient rights set out in the Act on Patient Rights and the Patient Ombudsman and keeps and provides medical records under that Act. Information: Patient rights.
11. Complaints
Complaints can be made as described on the Complaints page. The Practice replies within 14 days.
12. Refunds
Refunds are made within 14 days, in the payment currency and using the same payment method the Patient used, unless the Patient agrees to another method at no cost to them. Before disputing a payment with a bank (chargeback), please contact the Practice — this does not limit the Patient's rights. The provisions of the Polish Consumer Rights Act on distance contracts do not apply to health care services (Art. 3(1)(7) of that Act); regardless of this, the Practice applies the free cancellation rules in section 7.
13. Personal data
The Practice is the controller of personal data, including health data — see the Privacy policy.
14. Final provisions
These Terms apply from 30 October 2026. They do not exclude or limit Patient rights under mandatory law. Polish law applies to matters not covered here, in particular the Medical Activity Act, the Act on Patient Rights and the Patient Ombudsman, the Act on the Professions of Doctor and Dentist and the Civil Code. Visits booked before a change to these Terms are governed by the Terms in force on the day of booking.
