Complaints
If anything about your visit, the service you received or your bill concerns you — please tell us. We take every complaint seriously and reply in writing.
How to complain
Write to [email protected], send a letter to Indywidualna Praktyka Lekarska Katarzyna Zugaj, ul. Antoniego Dobiszewskiego 2, unit 23, 01-405 Warsaw, Poland, or make a complaint in person at the practice. Please include:
- your name and the date of the visit,

Response time
We reply within 14 days of receiving the complaint — in the form in which it was made, or another form you indicate.
